You must have all your natural teeth, excluding wisdom teeth. Required for a proper appliance fit.

Your physician must complete and sign the Slow Wave prescription before your DS8 can be manufactured. Print it, take it to your sleep doctor with your most recent sleep study, and return the signed copy with your application.
Your physician can also fax the signed Rx directly to Slow Wave at 830-220-5701.
